At the Good Shepherd Diabetes & Nutrition Center, we are committed to providing quality education and support to all people with diabetes.

Living with diabetes is not easy but learning about your diabetes and how to make healthy choices is the first step towards feeling great and living a long, healthy life.

Whether you’ve been newly diagnosed or have been living with diabetes for many years, we are here to help! Our Diabetes Care & Education Specialists provide personalized education and support to help you live well with diabetes. We offer group classes and individual sessions for patients with all types of diabetes. Our goal is to empower you with the knowledge and tools to make healthy lifestyle choices, allowing you to successfully manage your diabetes and achieve your health goals.

You may benefit from diabetes education if you:

    • have just learned that you have diabetes
    • are not meeting your treatment goals (A1c, blood lipids, blood pressure, weight, quality of life, etc)
    • have questions about how to manage your diabetes
    • have had a recent change in your treatment plan or medications
    • have been diagnosed with a new health condition

When you attend diabetes education classes you will learn how to manage your diabetes and receive support for making lifestyle changes. Topics include:

    1. What is diabetes?
    2. Nutrition care for diabetes
    3. Physical activity for health
    4. Monitoring your blood sugar
    5. Understanding your diabetes medications
    6. Preventing complications
    7. Goal setting and problem solving

At the Diabetes and Nutrition Center we also offer a variety of diabetes management services to meet your needs. Our services include:

    1. Continuous glucose monitoring
    2. Insulin pump starts and education
    3. Insulin/medication training and titration, in conjunction with your health care provider
    4. A1c screenings

What to Expect

A referral from your diabetes health care provider is required and insurance is billed for diabetes services. Financial assistance is available to help cover the costs of diabetes education.

    • If your diabetes health care provider has referred you, we will call you to set up an appointment after we have reviewed your referral and obtain insurance authorization as needed. This process typically takes 1-2 weeks.
    • If you are interested in diabetes education but your provider has not referred you, you may call our office and we can help request a referral from your provider.
