HOME MEDICATION LIST

Know your meds and keep a list with you.  Knowing what medication you are taking is important to doctors when they are diagnosing your problem and giving you treatment - including over-the-counter drugs, like aspirin, and "herbal" supplements.  Keeping a list with you protects you if you have an emergency, and can't tell or remember what is on your list.  Click here to print a tri-fold wallet form for your use.  This is an Acrobat Reader form.
