Home Health Care provides, with a physician's order:

Skilled Nursing

  • 24-hour, on-call Registered Nurse availability
  • Skilled assessment, observation and evaluation
  • Medication management and education with focus on pain management
  • Physical/systems assessments
  • Teaching of health care issues

Certified Home Health Aide

Under the supervision of a registered nurse or therapist, a home health aide assists the patient with prescribed self-help, including personal grooming, needs for bathing and ambulation.

Therapy Services

Licensed physical therapists provide treatments to relieve pain, restore body functions and mobility, and teach special use of devices to aid rehabilitation.

A registered/licensed occupational therapist helps individuals achieve independence in their lives despite disabilities.  Occupational therapy provides assessment and treatment in rehabilitation needs of self care, functional mobility, home management, vocational and leisure activities, and educational activities.  Occupational therapy assists people in mastering the "skills for the job of living" after illness, injury or disability.

A licensed/certified speech language pathologist provides evaluation and treatment in the areas of communication related to speech, language, word recall, cognition, oral function and hearing loss.  The therapist also provides evaluation and treatment of swallowing disorders, including safe swallowing techniques, texture management, and teaching compensatory skills.
