ConneXions:

•Number of patient encounters (in-person and phone):

•Certified Community Health Workers trained in several

specialty areas: Peer Wellness Specialist, Doula, Medical
Interpreter, Child Passenger Safety Technician, OHP
Assistor, and End of Life Doula (new in 2024).

•Continued Teen Mental Health Outreach program at

Hermiston High School with great success: weekly group
counseling sessions with ConneXions’ certified Peer
Wellness Specialist and HHS’s staff to over 20 teens, the
vast majority reporting an increase in mental well-being.

Community Health/Education:

•Number of classes/events offered: 286
•Potential reach of individuals via classes or events: 23,986
•Received a total of 8 grants for various community

health projects: expanding doula services, child car
seats, parenting classes, substance abuse awareness and
prevention, local teen health initiatives, and Family Health
& Fitness Day.

•Hosted annual community event, Family Health & Fitness

Day + 5K Fun Run, on September 23, 2023 with over 600
attendees and over 50 vendors.

•Hosted second annual You Are Not Alone Mental &

Behavioral Health Awareness Event and Honor Walk on
April 27, 2024 with over 300 attendees.

•Continued work on Good Shepherd’s 2022-2025

Community Health Improvement Plan and its 4 priority
areas: Mental Health, Obesity/Nutrition/Physical Activity,
Women’s/Men’s/Infant Health, Behavioral Health.

•Created and shared 26 community class/presentation

videos

•Attended 54 community events/health fairs
•Offered free flu shots at 10 community events, including 1

drive-thru event, totaling over 500 individuals vaccinated.

•Offered 3 Teddy Bear Clinics in the community with over

400 local children participating.

•Hosted 15 American Red Cross blood drives
•Performed 73 car seat checks/installations
•Number of Senior Health Insurance Benefits Assistance

(SHIBA) patient encounters, in-person and phone: 1,650
