Community Wellness

At Good Shepherd, we focus on wellness and actively promote activities that encourage people to live a healthy lifestyle. Our wellness coordinator provides wellness assessments for individuals, business employees, and groups.

Community Health & Outreach Department

Good Shepherd Medical Center is pleased to make available to patients, family, medical personnel, and the public in general a fully functioning Community Health & Outreach Department. The department consists of a wide variety of resource materials on health, wellness, or disease topics. Health information and wellness classes are run regularly at Good Shepherd. The Community Health & Outreach Department is located at 645 W Orchard Ave., Suite 200 and is open weekdays from 8:00am to 5:00pm.

SHIBA

The Senior Health Insurance Benefits Assistance (SHIBA) program is a statewide network of trained counselors who volunteer their time to educate and advocate for people of all ages who have Medicare.

In Good Shepherd Health Care System’s Community Health & Outreach department, there are multiple certified Medicare counselors working with the Oregon Senior Health Insurance Benefits Assistance (SHIBA) program. We see SHIBA clients on a daily basis either in-person, virtually, or via phone year-round. Medicare open enrollment for Medicare Advantage and Prescription Drug plans is Oct. 15 to Dec. 7. Don’t miss your opportunity to shop, compare and make enrollment decisions. Even if you continue to work or are not receiving Social Security, you need to know about Medicare to avoid penalties in your Medicare coverage. ConneXions

ConneXions_logoConneXions assists community members in getting connected to the many services offered throughout Umatilla and Morrow counties. The ConneXions team consists of certified, experienced Community Health Workers, as well as a highly trained nurse overseeing patient care. Community Health Workers work collaboratively with providers, services, and agencies to help our clients with health issues, basic needs, mental or behavioral health concerns, medical transportation, medication management, peer support, healthcare coverage, or financial needs. Using health education, screenings, and partnerships between our clients, their healthcare provider and ConneXions, our goal is to encourage healthier behaviors in our community members along with decreasing unnecessary healthcare service utilization.

The role of the Community Health Workers on the ConneXions team is to (1) provide culturally appropriate and accessible health education and information, (2) assure that people get the services they need, (3) provide informal counseling, mentoring and social support, (4) build individual and community capacity and (5) provide direct service to community members.

Referrals can be generated by anyone in the community: providers, community service agencies, self-referrals, EMS, Law Enforcement, from community outreach activities, well as internal Good Shepherd departments. Healthy Communities Coalition

Good Shepherd has taken the lead in forming and providing ongoing support for the Health Communities Coalition. The mission of the Coalition is to work together to promote a healthy community through education, support, and community partnerships. The Coalition is made up of community partners including representatives from police, fire, schools, healthcare organizations, area businesses owners, community leaders, and volunteers.

CareVan Medical Transportation

CareVan Medical Transportation travels to Hermiston, Echo, Stanfield, Umatilla, Irrigon, and Boardman. It is an Americans With Disabilities Act (ADA) compliant service provider. Rides are provided to community members to any Good Shepherd Health Care System affiliated medical or service provider with offices in Hermiston. Rides operate from 8:00 am to 6:00 pm, Monday through Friday. Click here to learn more about CareVan.
